How do I enroll?

New employees must request and complete the New HIre Benefit Enrollment Form indicating their elections to enroll or waive the benefit plan options and submit the form to their county office.  Designated contact will work with Extension  and Outreach and University Human Resources to enroll the new County Paid Employee.   New employee starting anytime during the month will have insurance coverage beginning on the first day of the following month.  

What happens if I waive insurance upon hire?

Your next opportunity to enroll will be in November for coverage effective on January 1st.  Or you may be able to begin coverage if you experience a qualifying event which would allow enrollment. The effective date and the changes that can be made must be consistent with the event itself. You have a limited amount of time to request a change due to a qualifying life event. Can I enroll in Medical and not Dental?

Yes, you may enroll in only medical insurance, only dental insurance, only flexible spending accounts, or a combination of any of them. A prescription drug plan, Express Scripts, comes with the Wellmark medical coverage so you do not enroll separately in this coverage.

How will I know I was enrolled?

If you enroll in flexible spending, you will get a welcome letter from ASI Flex. If you enroll in the medical/prescription or dental plans you will get those member ID cards within 2 to 3 weeks.

How can I check to see if my provider is in-network?

For your medical coverage, you can use Wellmark’s Find A Provider website. The plan prefix for HMO is XQW and for PPO it is IZU.

For your dental coverage, use Delta Dental’s Find a Provider webpage to search for a Delta Dental Network Dentist. Your plan is the Delta Dental Premier.

What is a Primary Care Physician and how do I find one?

This is a term associated with our HMO plan. To enroll in the HMO, you must designate a Primary Care Physician (PCP). The PCP must provide all routine and preventative services to HMO members. Use Wellmark’s Find A Provider website to search for a provider who is a designated Primary Care Physician currently accepting patients.

How do I change my Primary Care Physician?

To change your Primary Care Physician, please contact the Wellmark customer service number located on the back of your member ID card.

When can I make changes to my benefit elections?

All new employees have an opportunity to enroll upon hire.   You will also have an opportunity to make changes during our Annual Open Enrollment period each November. Changes made during Open Enrollment will go into effect January 1st of the following year.

How do I request changes to my benefit elections?

Contact the ISU Benefits Office to request changes in the middle of the year due to Qualifying Life Events.

The ISU Benefits Office will send you information annually about your Annual Open Enrollment Period and process.

Whom do I contact with questions?

If you have questions about your coverage, please contact the insurance companies (Wellmark, Delta Dental, Express Scripts, and ASIFlex) using the customer service numbers on your member ID cards.
